Renal Papilla
Renal papilla is the tip of each pyramid where urine drains into minor calyces.
The renal papilla is the tip or apex of each renal pyramid where urine produced in the kidney is funneled into the collecting system. It marks the transition from the medullary collecting ducts into the minor calyces. The renal papilla plays a crucial role in channeling concentrated urine out of the renal pyramid and into the renal pelvis for transport to the ureter.
Structure
The renal papilla represents the terminal portion of the renal pyramid and is composed primarily of:
- Collecting ducts of Bellini: Final drainage channels from nephrons that converge at the papilla
- Simple columnar epithelium: Lines the ducts and transitions into urothelium at the papillary tip
- Openings at the cribrosa: Numerous small pores called the area cribrosa where urine is discharged into the minor calyx
Location
Renal papillae are found at the innermost part of each renal pyramid:
- Projecting into the lumen of a minor calyx
- Located deep within the renal medulla
- Often surrounded by the renal sinus fat and close to the beginning of the collecting system
Function
The renal papilla serves as the final drainage site for urine within the kidney:
- Urine delivery: Delivers urine from collecting ducts into the minor calyces
- Funnel-like outlet: Focuses outflow from the pyramid into the calyceal system with minimal resistance
- Transition point: Marks the beginning of the urinary excretory tract beyond the nephron
Clinical Significance
- Papillary necrosis: Ischemia or injury to the papilla (e.g., due to diabetes, NSAIDs, sickle cell disease) may cause sloughing, hematuria, and obstruction
- Radiologic imaging: Renal papillae may appear blunted or irregular in chronic kidney disease or infection
- Urolithiasis: Kidney stones can originate near or become lodged in the area around the papilla, especially where minor calyces attach
- Anatomical variations: The number, size, and shape of papillae can vary between individuals and even between kidneys
The renal papilla acts as the anatomical gateway between the urine-producing nephrons and the excretory drainage system of the kidney. Its integrity is vital for unimpeded urine flow and proper renal function.
