The dorsal nerve of the penis is a significant sensory nerve in the male genitalia. It is one of the terminal branches of the pudendal nerve, which itself originates from the sacral plexus with contributions from the second, third, and fourth sacral spinal nerves (S2-S4). The dorsal nerve of the penis plays a critical role in the sensory innervation of the penis, influencing sexual arousal and pleasure.

Anatomy

The dorsal nerve of the penis branches off from the pudendal nerve, travels through the Alcock’s canal (also known as the pudendal canal), and then progresses along the penis on its dorsal side (the top side, opposite the urethra). As it runs along the dorsal aspect, it lies within the deep fascia of the penis, alongside the dorsal arteries and veins, reaching towards the glans penis.

Primarily comprised of sensory fibers, the dorsal nerve of the penis branches out as it extends along the length of the penile shaft and into the glans. Conditions such as Peyronie’s disease (characterized by fibrous scar tissue inside the penis) can also impact the function of the dorsal nerve. In surgical procedures like circumcision, penile surgeries, or the treatment of penile fractures, careful handling of the dorsal nerve is crucial to avoid postoperative complications related to sensory loss or dysfunction.
